So apparently it's not a bug, it's a security feature to disable the preview pane for any file that has been downloaded and Microsoft isn't changing it back.
If anyone has noticed they can't preview since an update a few months ago, here is the solution.
Right click the file, select Unblock.
Exit out of file explorer and it should now be seen in the preview pane.
The hassle with this is you have to do it for every damn file.

Chat GTP to the rescue... it created a file that I added to the registry so I can have "Unblock all files" on a right click menu.

Create this file in notepad (any text editor) and save it as Unblockallfiles.reg

Windows Registry Editor Version 5.00

[HKEY_CLASSES_ROOT\Directory\shell\UnblockAllFiles]
@="Unblock All Files"
"Icon"="powershell.exe"

[HKEY_CLASSES_ROOT\Directory\shell\UnblockAllFiles\command]
@="powershell.exe -NoProfile -ExecutionPolicy Bypass -Command \"Get-ChildItem '%1' -Recurse | Unblock-File\""

Then double click that file to merge it into the registry.
Bingo-Bango now you can unblock all files in a folder with 1 click.
The folder I do it on every morning has about 50k files of prints and shit, and it takes less than 10 seconds to do them all.
