a too hot blank load or live ammo .. it really doesn't matter.. he shouldn't had been
pointing the weapon at people

Movies that use firearms have onset armorers. It's that guy's job to make sure all weapons are safe and then hand them to the actor, who likely isn't qualified to make such a determination.

Back 20ish years ago when Bruce Lee's son got killed in a similar way, the investigation found the gun had previously been firing actual ammo, and the last bullet fired lodged in the barrel. When they put the blanks in they never bothered to check the barrel.

And  why even have a gun in a movie that can hold blanks or bullets. They add the sound

Weren't they AR-14 Fully Semi-automatic weapons they were mad about?
