After 40 years of programming my mouse hand is about worn out. I've been using a vertical mouse for the last 13 years but it no longer serves the purpose.
I would like an ergonomic mouse that is neither vertical nor a track ball.

Any suggestions?

Be honest, that's not the only activity that has worn out that hand over the last 40 years!

I won't get tired of shilling the 3Dx CadMouse Pro. I have the wired one, its incredible. Been my mouse of choice for a decade. Dedicated middle mouse button, and side nav buttons (forward and back), and a little rotary menu button to customize as a kind of secondary right click menu. If you want/need more of a physical button bank on the side, it won't fit the bill. Has a very nice sensor, durable, nice gliding feet. My current one bought at this workplace is ~4 years old, I have another two at home that are 8 and 10 years old.

Ditto. Have a couple corded ones at home. Running the wireless version at work now.

I'll probably just quit when my trusty old MX5500 key board dies. It's pretty much irreplaceable.

What do you like most about them? I'm more interested in comfort than bells and whistles.

The 3D Connexion Mouse is SUPER light. It doesn't have to carry around that battery and all the things to support that. 

I don't use that mouse but a Logitech MX Master. My hands are too big for the 3D Connexion mouse. 

Do you use a Space Device (Space Pilot, Space Pilot Pro, Space Navigator, etc...). I cannot say enough about them. Been using one for better than 20 years now and I won't do CAD/CAM without one. Even when I'm on an airplane... I'll suffer from the short duration of a flight using the mouse pad... but the Space Navigator... not even for 5 minutes will I CAD/CAM without it. 

I went from being WELL on my way to Carpal Tunnel Syndrome to ZERO pain in years. The pain started to go away from my mouse hand within a month or so of going to the SpaceNavigator.

+1 for the Logitech MX Master. It's the best ergonomics I've tried and love the side scroll. Mine has taken many falls onto concrete. I've had it for at least 4 years and the battery is still going, lasts months.

I like that it's a straightforward, performant mouse. I have never had fatigue or strain, using it for a 10 hour workday, and then gaming for a few hours at night.

I like that I have zero issues with it ever, it simply keeps on working.

I love the precision of the sensor, it has a remarkable balance of speed/accuracy: "LET'S HOOF IT ACROSS ALL THREE MONITORS" and just as easily making small 1 px movements without wondering when the cursor sill start moving.

That being said, everyone who sits down at my computer (even IT) immediately sends the cursor off into space and asks how I can "use it like this". That's with speed sliders in the middle on the 3Dx app, and windows settings. So, one negative point is all I have.

I ordered the Logitech MX (oddly enough, before I read James' post,  :) ). I was afraid the 3Dx mouse would be more function than fit.

I do have a Space Navigator. Not by choice; that's all that's available to me without buying my own. I, too, have been using a 3D mouse for many years.

My wrist has bothered me for years. Probably 15 years ago I had surgery for carpal tunnel; they also removed the pisiform bone from my wrist and relocated my ulnar nerve. None of it helped a damn bit. I'd been warned about the surgeon who did it. I wish I had taken head.

I have pain in my wrist, hand and elbow. I would like to keep going for another year or two but I'm not sure if I will be able to.
