Where's The Duct Tape??

Bolts holding the door up in place are missing.

Also the cockpit voice recorder was forgotten about afterwards , power never turned off so it was overwritten.  :popo:

The little bit I've read says there should be 4 bolts holding this plug in. 4 bolts, with a castellated nut, with a cotter pin. And the door would be secure with just one of the fasteners. So the odds of 4 cotter pins somehow falling out, 4 nuts rattling loose, and 4 bolts falling out of their holes at the same time...I am going to say are pretty slim. Betting they were never installed in the first place, atmospheric pressure was holding the door in place till it didn't.

So who installed the plug in the first place?

Spirit installed it originally. Boeing then removed it to aid in interior installation and reinstalled it.

Typically all aircraft fasteners have a marking paint (a line ) that is applied to the nut/bolt/washer/component to identify if anything has moved. Just wondering if this in place on these fasteners.

Shouldn't need them on Castlenuts/bolts. As long as the cotter pins are in they won't move. And the bolts are long gone, likely in the woods.

Ive always heard that called torque paint. Fastener gets torqued, you paint a line showing that. You see it all over Matsurra machine tools. I would bet there is a new procedure requiring this along with the cotter pin being secured to the fastener with a cable.

back in the day bolts and screws were secured with safety wire or cotter pins.
you couldn't unscrew a bolt without cutting the safety wire
the mechanics had to install the safety wire so that it put tension on the bolts so they couldn't come loose
It was a real art form and amazing to watch someone who was good at it.
I think they have nuts and bolt that no longer require saftey wire..... but they cannot be reused

Apparently, there is no official paperwork specifically stating that Boeing removes the panel for interior installation and then reinstalls it. Plausible deniability.

It may be Spirit's fault. With our recent history it wouldn't surprise me. My location is so far away from the production area I have no idea what goes on there. They sure talk about quality and safety a lot.
