the lathe guy was required to fill out a cause and corrective action report

the cause... a burr caused an incorrect measurement resulting in a bad offset adjustment for the finish pass

the corrective action .... reprogram the semifinish pass so there is no burr 

Really And they accepted that Pi$$ Poor Excuse.
That sucks G. 

On a side note it did make me laugh imagining Mowens Scooting across the room and going A$$ up 
Ive Missed so Much 
Its Ok Mowens. I was coming back from a 500km ride and was 2 minutes from home and had a car pull out on me. I found out that Harley's don't jump Round A bouts too well. 5 months since that happened and im still struggling to get back to any kind of normality. I have compression Fractures in my L4 and L5 vertebra and had bleeding around my spine for the first 2 months. Might take a bit longer to come back from this one.

I screwed up a piece of exotic stainless one time that cost a couple hundred bucks, but after hearing gcode and Jeff's stories I don't feel nearly as bad about it! 

Knowing your lathe guy measured a critical dimension without deburring it first is hard to take.

Our Manufacturing Manager died unexpectedly about 4 weeks ago at the age of 60.
He left behind a huge pair of boots to fill and we've been really struggling.
This is just one is a string on bad days lately.
